The Bravo Health Advanced Care Center (the Center) is designed to treat immediate and complex health issues.
Immediate health care issues: Treatment for injuries and illnesses
If you feel you need to see a doctor and are not able to see your own doctor, you may come to the Center for treatment. Your doctor may also tell you to come to the Center if he or she:
- Is not able to see you right away because the appointment schedule is full or you need care when the office is closed (i.e. after hours or on the weekends)
- Believes your health condition needs more time or resources than he or she can offer you in the office
You do not need to have an appointment to come to the Center for treatment.
The Center is open for walk-in visits.
Examples of immediate care issues that may bring you to the Center:
- Infection
- Dehydration
- Shortness of breath
- High/low blood sugar due to diabetes
- Minor injury, such as sprains, wounds, etc.

Complex care program
The Center is designed to be an extension of your doctor's practice so that you get the care you need, when you need it.
Examples of complex care issues that the Center can treat:
- Heart disease
- Arthritis
- Diabetes
- Pulmonary Disease (COPD)
- Asthma
The Center can also help you if you have been recently discharged from the hospital to assist you in your transition back home and in receiving appropriate and timely care to so re-admission may be avoided.
