Health Care for People Most In Need
Many patients with the greatest need for health care often seek treatment in the emergency room. People who repeatedly visit the ER for care may see few other alternatives. They may be medically underserved, have low incomes or face other barriers to finding the right health care for them, when and where they need it.
Improving quality of life for these patients requires offering access to immediate and complex care. The ideal solution should extend the primary care provider's services with a broader range of treatments than can be cost-effectively provided in the current health care delivery system.
While the health care system works well for the majority of the population, patients with immediate and complex conditions often need greater attention. These patients require special consideration to ensure that their health care needs are met.
Physician-directed Care at the Point of Need
Based on the premise that everyone deserves attentive, high quality health care delivered in a pleasant, accessible setting, the Bravo Health Advanced Care Center (the Center) is a physician-directed patient care delivery system designed to address immediate and complex health issues at the point of need. Serving as a link between the hospital and the primary care physician, the Center is an innovative facility equipped to deliver care beyond what is practical in most primary care physicians' offices.
The first of its kind in the region, the Center currently serves Bravo Health members (here and after referred to as "Patients") exclusively in the North Philadelphia community and is strategically located based on member concentration at 1010 W. Lehigh Avenue, Philadelphia, PA, 19133. Part gathering place and part medical facility, the Center offers Patients enhanced health care services in one convenient and comfortable neighborhood location. The Center also supports primary care physicians in the community by providing patients with a safe and reliable alternative to the hospital emergency department for the treatment of many non-emergent conditions.
Improving Overall Quality of Care
The goal of the Center is to improve overall quality of health care delivery and achieve optimal outcomes for every Patient it treats.
The Center works toward that goal through the following ways:
Safely treating patients in the ambulatory setting whenever possible.
Regular hospital admissions can be wearisome and stressful for patients. The Center reduces hospital admissions by working with primary care physicians and other specialists as needed to intensively manage the care of members with chronic conditions to prevent acute exacerbations.
Delivering appropriate care in the appropriate setting.
The Center reduces non-medically unnecessary ER visits by providing advanced primary care in the ambulatory setting.
Aligning preventive health and conformity with accepted treatment guidelines.
The Center performs a select number of preventative health evaluations and tests for members who are in need of these services. Such tests may include body mass index, cholesterol management, fecal occult blood test, annual medication monitoring, blood pressure control, and comprehensive diabetes care, among others. The Center also provides complete physical exams to capture all patient diagnoses, and reviews them each year as appropriate.
