Coordinating Complex Care with the Bravo Health Primary Care Physician
The Bravo Health Advanced Care Center (the Center) is a health care practice that addresses immediate and complex issues at your patient's point of need.
Exclusively for Bravo Health members, the Center is designed to support and broaden your practice's capabilities while offering a reliable alternative to the emergency room for non-emergent conditions in one convenient and comfortable neighborhood location. The Center is staffed by board certified physicians and features onsite radiology, laboratory, observation, and monitoring.
Immediate Care: Treatment for Injuries and Illnesses
- Friendly, accessible environment
- No appointment necessary
- Treatment for conditions such as infections, dehydration, shortness of breath, diabetes-related symptoms, and minor injuries
Complex Care Management
- A bridge connecting the primary care physician, multiple specialists, and the hospital
- Physical exams and treatment for conditions such as heart disease, arthritis, or asthma
- Eases the transition back home for recently discharged patients
For physicians participating in Bravo Health Pennsylvania Inc.'s voluntary Network Quality Improvement Program (NQIP), the Center provides additional benefits:
- HEDIS ® Measures Improvements: The Center will perform a select number of tests as necessary, including body mass index, fecal occult blood test, annual medication monitoring, beta blocker compliance post-MI, blood pressure control, cholesterol management, pharmacy therapy of COPD, anti-depression medication management, and comprehensive diabetes care (HbA1c, LDL-C)
- HCC Coding Enhancement: The Center will capture and document all clinical conditions using the appropriate ICD-9 codes
