You'll know the Bravo Health Advanced Care Center (the Center) is a special place as soon as you walk in the door.
When you get to the Center, our receptionist will greet you, gather all the necessary information for the visit, and make sure you are as comfortable as possible. A relaxing seating area with a TV, as well as light snacks and drinks, are available.
You will be seen by one of the Center's health care professionals who will arrange for any diagnostic and treatment services as needed.
If your doctor suggests you come to the Center for a complex health issue, the Center's staff will discuss and review your medical history with your doctor. Your doctor may request that you have a physical exam as a new patient at the Center.
After your visit, the Center will give your doctor a detailed report about your treatment as well as an update on your condition.
Remember:
- There is no copay for services provided by the Center's staff, in accordance with your Bravo Health benefit plan.
- For immediate health issues, no appointment is necessary.
- Make sure you bring all your medications with you in a bag for your visit.
