What to Expect When Your Patients Visits Us

The Bravo Health Advanced Care Center takes a team approach when treating patients by coordinating complex care with you, the primary care physician.

Complex Care
Our complex services are available from 7 am to 4 pm, Monday through Friday. Our board certified internist with extensive geriatrics experience will engage you in a clinical discussion of the patient's condition and needs and request the pertinent data from the patient's medical record.

We will then schedule the patient for an initial consultation and develop a care plan in conjunction with the Bravo Health Case Management Team. We'll schedule an update every two weeks to keep you abreast of the patient's progress and solicit your input into the care plan. You have the option to charge the patient's Bravo Health managed care plan, Bravo Health Pennsylvania, for the bi-weekly consultation. To learn more, please contact your Provider Account Manager.

Immediate Care
Our immediate care program services are available from 9 am - 9 pm, 365 days of the year. Your patients can come to the clinic and receive prompt, courteous, and comprehensive care. Whether your patients come to the Center on their own or you refer them due to your busy schedule, we'll send you a report of their condition and treatment plan generated from our Electronic Medical Records (EMR) system the very next business day. We will also make a follow-up appointment with you on behalf of your patients.

We will have staffing, diagnostic testing, and medical equipment to provide a wide variety of services that can be conducted safely in the outpatient setting. Some examples of the services we have are:

  • Intravenous therapy
  • Continuous, non-invasive vital sign monitoring, including cardiac monitoring
  • Oxygen therapy
  • Digital plain film radiology
  • Suturing
  • Areas for extended treatment

Our on-site pharmacy assures your patient has access to the appropriate medications so that their treatment is already underway when they return to your care.
