The Bravo Health Advanced Care Center offers patients enhanced health care services in one convenient and comfortable location. Serving Bravo Health members exclusively in North Philadelphia, the Center is an innovative, state-of-the-art physician practice equipped to deliver extended treatment services.

Featuring in-suite radiology, laboratory, observation, and monitoring, the Center is staffed by physicians who are Board Certified in Internal Medicine, with significant experience in the management of chronic conditions in the Medicare population, as well as Board Certified physicians skilled in tertiary care.

As a physician-directed patient care delivery system, the Center is designed to address patients' immediate and complex health issues at their point of need:

- Immediate Care: Treatment for Injuries and Illnesses
The Center is able to treat a patient's immediate care needs in a friendly, accessible environment. No appointment is necessary - Bravo Health physicians are able to immediately direct their patients to the Center for the care of such conditions as illnesses, infections, dehydration, shortness of breath, diabetes related issues, and minor injuries. Since the Center's staff are dedicated to serving only Bravo Health members, a visit to the Center offers patients comprehensive yet prompt treatment.

- Complex Care Management: Advanced Primary Care
With a broader range of treatments than is typically provided in a primary care practice, the Center is designed to be a bridge between primary care, multiple specialists, and the hospital for patients with chronic health conditions. The Center invites members with complex conditions such as heart disease, arthritis, or asthma to visit the Center for a new patient physical exam. Physicians will also assist recently discharged patients to ease the transition back home, assuring appropriate and timely care to avoid re-admission.
